B&W, and increase the focus on the face
Apply a black and white lens filter effect, and slightly increase the red in the color balance sliders.
Apply a vignette effect around the border of the image, and modify the level, width, and height to fit the focal object.
Use the masking tools to define the background, then apply a DoF effect with a custom gradient and sharpen the foreground which contains the subject image.
