Altered character recall
Character created in CC3 and later saved. When recalled, it appears altered. I have seen this type of defect that is proposed with one type of shirt and not another. I have performed the following actions suggested by your assistance but without positive result:

"I would suggest:

Run Windows Update.

Install the most recent driver for your graphics card.

reinstall the content pack if you have not already.

reinstall CC3 using this clean method. Your content folders are safe during this process.

Uninstall Character Creator 3 and CC3 Pipeline Extension.

Check this directory for any CC3 leftovers:

C:\Program Files\Reallusion

Find this directory: C:\Users\your name\AppData\Local\Temp

Delete this folder: CharacterCreator3Temp

Note that AppData is a hidden folder, and you may need to set "Show hidden files and folders" for Windows

Also note that "your name" is your user name on your computer.

Download and unzip this registry cleaner. Double-click to run it. If it does not find anything, then Windows took care of it.

This is optional. If you want to be sure your system is as clean as can be, you may want to download and run Jetclean. It's a registry cleaner.

Install the current version of CC3 and CC3 Pipeline Extension from the Hub, or downloaded from your Reallusion Account. Install both parts before running."

Did you modified the skin weight of this T-shirt before you apply to character?

Yes

If you apply the Plain Base in Template tab then save the project, did this project still occur the same issue?

What do you mean by Plain Base in Template?

If problem still exist, please provide this folder within your template install path : ...\Reallusion\Template\Character Creator 3 Template\ContentPatch, also your plain base T-shirt.

You must excuse me but it is not clear what I should do.

Q1 : What do you mean by Plain Base in Template?
Template is where you install this content. Cause we use the same T-shirt but can't reproduce the issue.
So, I want to check if the original T-shirt in your template folder doesn't have the issue or not.

Q2 : You must excuse me but it is not clear what I should do.
Please go to template folder, the default install route is ( C:\Users\Public\Documents\Reallusion\Template\Character Creator 3 Template )
And find the folder calls "ContentPatch", then zip this folder and upload to cloud service such as Google Drive, and attach the download link in your next comment.
You can also set that comment as private.
